This portion is page heading. It has a page title. On the right side there is a unsubscribe job 
alert button.

Fields Area
This portion has all the fields of a job alert, the fields are

Name: Name of the user.
Category*: Category for job alert.
Contact Email*: email address on which notifications will be sent
City: City for job alert
Zip Code: Zip code for job alert.
Keywords: keywords for job alert.
Alert Type*: Alert type (daily, weekly, monthly).
Captcha: Captcha is only for visitor.

Category, Contact email, City, Zip Code, Keywords these fields are used for defining criteria
for job alert.
Alert type is how often job alert email will be sent. daily, weekly, monthly.
Daily means all jobs of a day will be sent.
Weekly means all jobs of a week will be sent.
Monthly means all jobs of a month will be sent.

Fields titles that have '*' next to their title are required.

Save Button

This portion has a save button. If all the required fields are filled click on save job alert 
button will save job alert. 

When a logged in user who has subscribed job alert opens this page again he will see all his provided details 
filled in the fields and click on unsubscribe job alert button in the heading will unsubscribe job alert.

If visitor wants to unsubscribe his job alert he will open this page and click on unsubscribe
job alert button then a pop-up will appear on screen that will have a field for email and button 
to unsubscribe job alert.

When user fills email field and clicks on unsubscribe button job alert associated with provided 
email address will be unsubscribed.
